Fall Prevention Program for Older Adults

“On Your Feet” is a specialized fall prevention program designed for adults aged 60 and older who are at risk of falling or have experienced a recent fall. By providing education and targeted interventions, the program aims to reduce injuries and promote independence, enhancing participants’ quality of life.

Participants must meet the following criteria:

  • Be 60 years or older.
  • Have experienced a previous fall or have a fear of falling.
  • Be able to walk, with or without assistive devices such as walkers or canes.
  • Live at home or in an independent or assisted living facility.
  • Have the cognitive ability to engage in program activities.

The “On Your Feet” program features three main areas of focus:

Fall Risk Assessment

  • Identifying individual fall risks.
  • Reviewing medications that may contribute to fall risks, such as sedatives or blood pressure medications.

Mobility Planning

  • Providing tips to maintain independence.
  • Offering guidance on the effective use of assistive devices.
  • Introducing exercises tailored to improve balance and flexibility for various fitness levels.

Home Safety Checklist

  • Detecting fall hazards in the home environment.
  • Offering advice for creating a safer living space.
  • Recommending simple modifications to enhance safety and mobility.
